HEALTH CLUB MEMBERSHIP PROCEDURE

Health Club applications may be obtained at the Personnel Window.
Take application to qualified Health Club to be filled out. You must have an authorized signature and paid in full receipt from Health Club to be reimbursed. .
Attach original paid receipt to application form. (Employee should keep copy, originals will not be returned)
Return application with attached receipt to Payroll Window for reimbursement.
. Reimbursement will be included in your payroll check.
. Form must be received by noon on Thursday for reimbursement the following week, under
normal circumstances.
Employee will be reimbursed for the cost of a yearly membership up to a maximum of $500 per year or pro-rated at $42 per month.
Qualified period began May1, 1997, and was modified to the current agreement on March 11, 1998.
Existing memberships can qualify for reimbursement at the higher rate provided they were obtained on May 1, 1997 or later'.
If an employee has an existing membership with a health club, they can be reimbursed for the months beginning May 1,
1997 through the end of the health club contract with a paid-in-full receipt for a maximum up $500 or a pro-rated amount of $42 per month.
If the yearly membership cost at an area health club is less than $500, General Motors will only reimburse that amount.
Applications for health clubs must be in the name of the active employee.
.Employee can choose to pay Health Club Membership on a monthly basis, however, payment will not be reimbursed until contract is
paid in full.
